“Community as Classroom”: University of Phayao Deploys Grassroots Innovations and Serious Gaming to Intercept Stroke Threat
Nursing students and faculty partner with rural northern communities across six subdistricts, equipping frontline Village Health Volunteers with digital board games to secure the crucial 4.5-hour golden window.
In a proactive demonstration of service-learning and public health mobilization, the School of Nursing at the University of Phayao (UP) has expanded its community-anchored initiative across six subdistricts in Phayao Province. Working in close collaboration with primary healthcare centers and local government bodies, nursing faculty and students are tackling one of Thailand’s most pressing non-communicable health challenges: Stroke—a leading cause of sudden mortality and long-term disability among rural elderly populations.

The academic outreach spans six diverse subdistricts: Champa Wai, Rong Kham Luang, Mae Sai, Mae Na Ruea, Ban Tom, and Ban Sang, engaging over 200 local residents, village leaders, and primary health workers. Moving beyond conventional classroom lectures, the program tasks nursing students with conducting field health assessments, analyzing unique village lifestyles, and co-creating context-driven “Appropriate Technologies” to directly resolve community health hurdles.

Grassroots Health Innovations Tailored to Rural Lifestyles
Through active village immersion, student-led initiatives produced practical, high-impact innovations:
- Blood Flow Model: An interactive visual tool illustrating vascular hemodynamics, making the complex pathophysiology of arterial blockage and stroke mechanics intuitive and easily understood by elderly villagers.
- Sit Fine & Spine Align: Ergonomic guidelines and physical relief routines specifically designed for shallot-trimming workers—a predominant agricultural livelihood in the region prone to chronic musculoskeletal disorders.
- TEAM-V Plus: A supportive caregiving guide and cognitive intervention framework tailored for rural families managing dementia and elderly relatives.
- Upstream Waste-Wise: A community waste-sorting platform linking environmental sanitation directly with village-wide preventive healthcare.
Gamifying Preventive Care: Empowering Village Health Volunteers as “First Doctors”
Recognizing that primary disease prevention relies heavily on local community networks, the initiative placed special focus on upskilling Village Health Volunteers (VHVs)—the trusted frontline healthcare workers in rural Thailand.
1.“Scan Kem” (Salt-Scan) Board Game
Conducted in collaboration with Tambon Health Promoting Hospitals (HPH) in Mae Sai and Champa Wai, this hands-on board game transforms dry nutritional guidelines and hypertension control into an engaging simulation. VHVs learn how to assess hidden dietary sodium, enabling them to guide villagers in adjusting their food preparation habits.
2. Digital Board Games for Stroke Detection
Under the “1 Faculty, 1 SDG” project led by Asst. Prof. Dr. Chanita Praditsathaporn, 36 frontline volunteers gathered at the Champa Wai Sub-district Administrative Organization. The workshop utilized digital board games to train volunteers in rapid stroke recognition and emergency dispatch workflows.

Beating the Clock: The Critical 4.5-Hour Stroke Golden Window
With chronic hypertension, diabetes, and hyperlipidemia frequently undiagnosed in rural areas, delayed hospital arrival remains a critical risk factor. The University of Phayao team emphasized key warning signs aligned with the global FAST framework:
- Face (Facial Droop): Sudden numbness or asymmetry of the mouth or face when smiling.
- Arm (Limb Weakness): Inability to lift one arm or sudden weakness in one side of the body.
- Speech (Slurred Words): Sudden speech impairment, confusion, or difficulty finding common words.
- Time (Every Minute Counts): Immediate transport to the nearest acute stroke-ready hospital within 4.5 hours from symptom onset to preserve brain tissue.

Core Preventive Measures Shared with Villagers:
- Routine blood pressure monitoring (as hypertension is largely asymptomatic).
- Reduction of sodium intake by limiting seasonings, preserved fish, and processed foods.
- Strict management of underlying chronic conditions.
- Regular physical activity while avoiding tobacco and heavy alcohol consumption.
